For Referring Physicians:

Patient satisfaction is vital in establishing credibility as a good health care provider.  This not only applies to the one-on-one care you provide in your practice, but also to the referrals you choose to make on behalf of your patients.  Patients who are referred to our facility to investigate their sleep disorder are genuinely happy with the care they receive and very impressed with our clean, modern facility.

Although we may be a new facility from a construction point of view, our medical and technical team consists of some of the most experienced people in the industry.  Our Medical Director is none other than Dr. Victor Hoffstein.  Dr. Hoffstein is one of the world’s leading authorities on snoring and sleep apnea and has published countless papers and articles over a span of more than 25 years, related to sleep medicine.

Our Technical Consultants, David Anderson and Laura Seed, bring over 35 years of combined sleep experience, including over 12 years in management in prominent Ontario sleep laboratories. We are proud to boast that at least one registered polysomnographic technologist (RPSGT) will be on duty at all times.  The stringent testing and experience that is required to become a RPSGT, ensures we always have highly skilled, BCLS certified, staff to take care of your patients.

Please click here to download our referral form and use it to refer any patient who may be experiencing signs/symptoms of a possible sleep disorder, including excessive daytime sleepiness, loud snoring, witnessed apneas, restless legs, insomnia, sleep walking/talking, etc.  All you need to do is complete the simple form and fax it to us and/or have your secretary call us at 416-642-4232 while the patient is still there at your office.  We will then book the sleep study appointment and provide all the necessary information.  If you have any questions, please feel free to contact us at 416-642-4232.
Thank you!
